What are the early warning signs of project failure?

What are the early warning signs that a project manager should notice prior to project failure? Can someone give me an exhaustive list.

I can think of a few warnings signs:

- Project is over budget.
- Project is behind schedule, and the delay from the original schedule variance is increasing every day.
- Team members are frustrated.
- Stakeholders are indifferent.

James Lewis, an American Project Management author suggests there is a 15% rule.  If the project is 15% into its timeline and is over budget or schedule (chances are it is still in planning), the project will remain in trouble unless it is replanned or otherwise recovered.
