How do you deal with unsatisfactory quality performance?

How do you deal with unsatisfactory quality performance? Could someone please describe the actions you take to identify causes and ensure a timely response is made once non-conformance is identified.

In short, if quality standards are not satisfied, then the project manager must take corrective measures immediately (such as scheduling and prioritizing bug fixing, etc...)

For major issues with quality, the project manager must inform the stakeholders as re-baselining might be necessary.

Personally, I think if you reach a point where you need to inform the stakeholders, then either the client requirements weren't gathered properly or there was no real quality management done during the course of the project. In any case, it means that the project manager didn't do his/her job properly.
