Assuming that you had been taken on as Project Manager for the Therac 25 development project, what would you have done to avoid its tragic outcomes?

Investigations revealed that defective software was among the various factors leading to this accident. This quite related with quality issue, thus quality related process e.g. Quality Assurance, Quality Control should put more into consideration.

While I don't think that the accidents with the Therac 25 project were related to bad project management, I do think that some actions from the project manager's side would have prevented them. Couple of actions I can think of:

- Better quality control: Apparently the quality control on the Therac 25 project wasn't top notch. A better quality control would have prevented some accidents.

- Better documentation: Obviously, the project wasn't documented properly and that's why the patients were given accidental overdoses.

Again, the problem with the Therac 25 is not really a PM problem. It has more to do with product management.
