While I don't think that the accidents with the Therac 25 project were related to bad project management, I do think that some actions from the project manager's side would have prevented them. Couple of actions I can think of:
- Better quality control: Apparently the quality control on the Therac 25 project wasn't top notch. A better quality control would have prevented some accidents.
- Better documentation: Obviously, the project wasn't documented properly and that's why the patients were given accidental overdoses.
Again, the problem with the Therac 25 is not really a PM problem. It has more to do with product management.
