I want to know who should be responsible and accountable when the project fails? Should the accountability be thrown on the project manager or the project sponsor? Is responsibility equivalent to accountability in this context?

Usually the project manager and/or project sponsor.  See the answer to http://projectmanagementquestions.com/409/who-is-accountable-for-the-project for more details.
