What happens during the hand-over phase?

I want to know what are the activities that take place during the hand-over phase. In all fairness, I don't know what the hand-over phase really is and it would be nice if someone can also explain it to me. Could it be that the hand-over phase is exactly the same as the project closure phase? Or is it that the hand-over phase is merely about handing over the project to another project manager?

The handover phase is the same as the project closure phase (it has nothing to do with handover process from one PM to another). Here are the activities that happen during this phase:

- Scope verification: All the deliverables listed in the project plan are checked against the project plan.
- Contract closure: The contract is formally closed after conducting an audit and ensuring that everything was done according to the agreed upon contract.
- Administrative closure: Project is officially closed from the company's side and team members are released.
- Lessons learned report creation: The lessons learned (that should have been gathered throughout the project) are formulated into a report.
