How can project communication be improved?

Since 90% of project management is about communication, I think it's very important to know what can a project manager do to improve communication in his projects? Is it by holding regular meetings? Is it by using a PM tool? Is it by creating a solid project communication plan?

A solid communication plan would be the best first step.  It would identify all the stakeholders, their communication needs and preferences, and the messages and information that needs to be communicated.  Outside of the communication plan, there should be generous amounts of informal communication as well. When there is an issue, notify the interested parties.  When there is a success, celebrate it.
