RACI Example

Can someone provide me with an easy and clear example on the RACI (Responsible Accountable Consulted Informed) matrix? I would prefer if this example is taken from a real project.

As the answer below states, it is very hard to give an example of a RACI with no attachment (unfortunately this website does not allow any).

Anyway, the first vertical column of a RACI lists all the tasks that the different stakeholders are interested in, and the top horizontal row (the header) contains the name of the stakeholders. In every cell intersecting between the row of the task and the column of the stakeholder, you should have one or more of the letters R (for responsible), A (for Accountable), C (for Consulted), and I (for Informed). For example, if in the cell intersecting between the task "Monitoring Project Schedule" and the stakeholder "Project Sponsor" you have "R,A", it means that the project sponsor is both Responsible and Accountable for monitoring the project schedule.

It's difficult to include a RACI without attachments.  You may find the following article of interest however:  http://blog.theprojectnotebook.com/2009/05/expectations-raci-and-raci-vs/.

I found an example here that is pretty complete, although not necessarily from a real project: https://wiki.service.emory.edu/display/public/Sample+project+RACI.

A Google search turns up at least 8-10 pages of results.
