What are the mistakes that companies make when closing out projects?

What are the mistakes that companies usually make during the project closure? I can think of three myself:

- Not having a formal celebration
- Not thanking the team members
- Ignoring the lessons learned

Are there any more?

I think you've covered the basics, here are more thoughts on the subject:

- Not formally closing out contracts
- Not assessing the performance of the team and the performance of the project manager
- Not communicating project closure to all interested parties
- Not adding the lessons learned to the company's repository (the repository should be controlled by the PMO)
