Where in the project lifecycle is it most appropriate to review your processes for effectiveness and why.

It depends if you're reviewing your processes for fixing the problem (in case of a problem) in your current project, or if you're doing it for future projects.

If you're reviewing the processes for effectiveness to take actions in the current project, then this should be definitely be done in the execution phase, but I'm not sure how easy it is to do that, without making the problem bigger.

If you want to do it for future projects, then you should do in the closing phase while doing your lessons learned.
