How does a RACI Diagram look like?

I want to know (from a professional) how a RACI diagram will look like, especially in production support (Communication, escalation, etc...)

I can tell you how a generic RACI should look like: it should have 2 headers, horizontal (on the top), and vertical (on the left). The horizontal header will display the roles (for example, Project Manager, Project Sponsor, Functional Manager, etc...), and the vertical header (on the left) should display the task.

Now in each cell intersecting between a role and a task (think about it as an Excel sheet), you add a letter (or more) describing the responsibility of the role on that particular task. That responsibility can be R for responsible, A for accountable, C for Consulted, I for Informed. Again, each cell might contain more letters, for example, CI means consulted an informed, etc...
